Resolve Agent Skills

Resolve all skills for an agent (including inherited from ALL environments and team).

Inheritance order (with deduplication):

1. All agent environments
2. All team environments (if agent has team)
3. Team skills
4. Agent skills

Later layers override earlier ones if there are conflicts.
